Solutions for an efficient visit

Preparing for your visit

Please arrive 10 to 15 minutes before your scheduled appointment time.

Bring the following to your appointment:

 • Insurance card

 • Patient Forms (Print our patient forms packet and complete them prior to your appointment)

 • A list of all medications

 • A list of personal health concerns or questions

Payment Policy

Payment is due when service is rendered. If you would like to inquire about special payment arrangements, please call the Billing Office directly at 810-629-7162.

For your convenience, the Billing Staff is available to answer any questions you may have in regards to your insurance plan. All non-contracted unpaid insurance claims and outstanding balances are you obligation. You may contact the Billing Office Monday, Wednesday, Thursday, and Friday 9:00am – 4:30pm at 810-629-7162

Emergencies

Should an emergency situation arise during normal office hours, please come directly to the Fenton Medical Center for assistance.

In the event of a life threatening emergency such as an automobile accident or a heart attack, we advise that all of our patients go directly to the nearest Hospital or call 911.

If you have an emergency after hours, please call 810-287-1114. One of our Physicians will be on call 24hrs a day, including weekends and Holidays. Please remember when using this number that you will be speaking with the Physician on call.

Appointments and Scheduling

For your scheduling convenience, our office is open Monday through Friday from 8:00am until 5:30pm. Routine appointments may be scheduled as early as 7:40 am.

It is our desire to accommodate all requests for visits with the Provider of your choice. However, should we be unable to meet those requests, we may ask if you wish to see another Provider that is available.

Should it become necessary for you to cancel an appointment, we ask you to call as soon as possible so that we may accommodate other patients.

Should there be any change in your name, address, telephone number or insurance coverage, please notify the receptionist so that your medical record can be updated.

For new patients, the receptionist will assist you with completing the necessary paperwork so that a medical record can be established for you.

HMO's and PPO's

We strive to accommodate patients who need urgent care when we are closed. Please call our Physician On Call to see if we can help guide your care in the evenings and weekends at 810-287-1114. If you have a non life threatening emergency you can go to our Urgent Care located in our building.

For Your Urgent Care Needs

The majority of health care insurance plans now require you to have your Primary Care Physician manage your entire health care. This means that a letter of referral is often required from the Primary Care Physician when services not performed by your Primary Care Physician are needed. Remember, these services will only be covered if they are included in your insurance benefit package.

For any referral questions, you may contact our Referral Department directly at 810-629-7066 Monday, Wednesday, Thursday, and Friday 9:00am – 4:30pm.
